At Ida’s House of Vermont in Brandon, we know that transportation is one of the biggest barriers for families caring for a loved one with dementia. That’s why we’re launching Lyght Rides, a dedicated transportation initiative serving only Brandon and Rutland residents.

Our Mission
Lyght Rides will provide safe, reliable, and compassionate transportation for:
  • Individuals living with dementia in Brandon and Rutland attending our day program, giving caregivers much-needed respite.
  • Residents of Ida’s House in Brandon, ensuring access to medical appointments, local outings, and emergency services.

Building Our Capacity
With community support, we will purchase a lightly used passenger van equipped with handicap-accessible features. This will ensure that residents of Brandon and Rutland—regardless of mobility—can safely access Ida’s House programs and essential services.

Why This Matters
For caregivers, even a few hours of respite can help prevent burnout. For people living with dementia, access to safe community programs reduces isolation and promotes dignity. With Lyght Rides, we’re ensuring that no family in Brandon or Rutland has to face these challenges alone.

The Healthy Horizons Project is an initiative designed to address healthcare disparities for older adults and low-income individuals in rural areas.
By integrating telemedicine and in-person care, we provide a comprehensive, flexible, and scalable approach to healthcare that is tailored to the unique needs of rural populations.

What is the Healthy Horizons Project?
The Healthy Horizons Project is inspired by the PACE (Program of All-Inclusive Care for the Elderly) model, which emphasizes coordinated, team-based care to meet a patient’s full range of needs—medical, social, emotional, and functional.

However, our project expands beyond the traditional PACE model to include:
  • Telemedicine and Mobile Health Units: Reaching patients directly in their homes and communities, providing comprehensive care without the need for long-distance travel.
  • Community-Based Partnerships: Collaborating with local organizations like the Chronic Care Initiative, Support and Services at Home (SASH), and the Vermont Office of Area Aging to create a seamless continuum of care.

By combining telemedicine with in-person services, we promote independent living while reducing reliance on emergency room visits and high-risk medications. This dual approach not only improves health outcomes and enhances the quality of life but also significantly reduces healthcare costs for Vermont’s most vulnerable populations. Our latest initiative, Enhancing Dementia Care in Vermont Nursing Homes through Specialized Support and Digital Coaching, aims to revolutionize dementia care in nursing homes across the state. Through advanced digital healthcare solutions and continuous staff support, we seek to provide compassionate, high-quality care for residents living with dementia.

What is the Dementia Care Initiative?
The Dementia Care Initiative is a pilot project designed to significantly enhance the quality of dementia care in three to five nursing homes in Vermont. By integrating advanced digital coaching platforms and providing continuous support for nursing home staff, we aim to improve patient outcomes, increase staff satisfaction, and elevate overall care quality.

Key Goals and Objectives:
  • Integrate Dementia Care Specialists: Hire and train specialized professionals to support nursing home teams.
  • Implement Digital Coaching and Training: Develop and provide a secure virtual platform equipped with advanced analytics for continuous staff education and support.
  • Reduce Burnout and Improve Retention: Enhance staff retention and satisfaction by reducing burnout and promoting skill development.
  • Improve Patient Outcomes: Focus on reducing adverse events and effectively managing dementia-related behaviors to improve the quality of life for residents.

According to recent studies, understaffing in nursing homes can significantly compromise the quality of care for over a million elderly residents. Our initiative addresses these critical challenges by providing continuous digital support and coaching for staff, ensuring the well-being of both residents and caregivers.
